Abstract
Chronic kidney disease (CKD) causes a progressive decline in renal function and affects multiple aspects of health, including sleep quality. Patients with CKD undergoing hemodialysis are at high risk for sleep disturbances due to physiological and metabolic changes, anxiety, and treatment-related fatigue. Acupressure is a nonpharmacological intervention that can be applied to manage this condition. Recent evidence shows that acupressure significantly reduces Pittsburgh Sleep Quality Index (PSQI) scores, as demonstrated by a meta-analysis by Cerit et al. (2023), which reported improved sleep quality among hemodialysis patients (p < 0.001), and a study by Amini et al. (2023), which found reductions in PSQI scores and anxiety after two weeks of acupressure therapy (p = 0.002). In this study, nursing care incorporating acupressure was applied to two CKD patients experiencing sleep disturbances: one managed patient (5 sessions) and one resume patient (4 sessions). The intervention was administered before hemodialysis by applying firm but comfortable circular pressure for 5 minutes at each point— Shenmen (HT7), Neiguan (PC6), and Yongquan (KI1)—preceded by deep breathing exercises. Assessment using PSQI indicated decreased scores in both patients, from 14 to 11 in Patient I and from 10 to 8 in Patient B, along with reports of improved sleep continuity and reduced fatigue. Acupressure proved to be effective, safe, simple, and feasible to teach to family members as a home-based supportive therapy. It is recommended as an evidence-based nursing (EBN) intervention supported by training and the establishment of acupressure standard operating procedures in hemodialysis units to improve quality of care sustainably.
