Abstract
Objective. Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by persistent systemic inflammation with various complications, one of them is osteoporosis. This study aimed to analyze the associations of age, body mass index, steroid dosage, and disease duration with neutrophil-to-lymphocyte ratio and vitamin D levels in patients with SLE. Method. We conducted an analytical cross-sectional study involving 40 patients with SLE at Fatmawati General Hospital, Jakarta, between January–December 2025. Secondary data from medical records were used to assess associations between age, BMI, steroid dosage, and disease duration with NLR and vitamin D levels. Bivariate analyses were performed followed by multivariable logistic regression. Result. Steroid dosage was significantly associated with NLR (p = 0.038) and remained the strongest factor in multivariable analysis (OR = 4.636; 95% CI: 1.023–21.004; p = 0.047). Disease duration was significantly associated with vitamin D levels in bivariate analysis (p = 0.022) and remained significant in the multivariable model (p = 0.029). No significant associations were observed between age or BMI with NLR or vitamin D levels. Conclusion. Based on the study findings, steroid dosage and disease duration as risk factors for osteoporosis significantly associated with changes in NLR and vitamin D levels. These findings are expected to inform clinical decision-making regarding the evaluation of inflammatory status and bone metabolism as part of strategies to prevent long-term complications in patients with SLE.
