Abstract
Introduction: Optimal vascular access is a key element in the success of hemodialysis therapy, as it affects patient morbidity and mortality. Although primary patency of arteriovenous (AV) shunts has been reported to reach 87.7% at eight months postoperatively, approximately 25% of patients still experience maturation failure. To date, no definitive predictor of post–AV shunt maturation failure has been established. This study aimed to evaluate preoperative, intraoperative, and postoperative risk factors that may influence AV shunt maturation failure and to identify predictors of maturation failure following AV shunt procedures. Methods: This retrospective cohort analysis used medical records from patients who underwent arteriovenous shunt procedures at RSUP Persahabatan between 2022 and 2024. The study endpoint was the identification of factors that predict post–AV shunt maturation failure. Patient characteristics analyzed included age, sex, body mass index, history of hypertension, diabetes mellitus, dyslipidemia, anastomosis site, and anastomosis technique. Results: A total of 120 patients met the inclusion and exclusion criteria; postprocedural AV shunt maturation failure occurred in 50 patients (42%). Univariate logistic regression analysis demonstrated that sex (p = 0.046) and history of diabetes mellitus (p = 0.027) were significantly associated with maturation failure. Female sex increased the risk of maturation failure by 2.12-fold (OR = 2.121; 95% CI: 1.013–4.441), whereas uncontrolled diabetes mellitus increased the risk by fourfold (OR = 4.083; 95% CI: 1.241–13.431). Conclusion: Preoperative characteristics, specifically sex and diabetes mellitus, are predictors of post–AV shunt maturation failure.
