Abstract
Background: HIV/AIDS remains a major global health challenge, with tuberculosis (TB) being the most common opportunistic infection. TB-HIV coinfection accelerates disease progression and worsens hematological and immunological parameters. Antiretroviral therapy (ARV) is essential for immune restoration, but evidence comparing its effects between HIV/AIDS patients with and without TB coinfection in Indonesia remains limited. Methods: A retrospective cross-sectional study was conducted using medical records of HIV/AIDS patients receiving ARV therapy at RS Pelni during 2022-2024. A total of 53 patients were included through total sampling method: 19 patients without TB coinfection and 34 patients with TB coinfection. Data on hemoglobin, platelets, total leukocytes, and CD4+ count were collected after six months of ARV therapy. Statistical analysis used independent t-test and Mann-Whitney U test depending on data distribution. Results: After six months of ARV therapy, patients without TB coinfection showed significantly higher hemoglobin levels compared to those with TB coinfection (p = 0.001). However, there were no significant differences in platelet count (p = 0.767), total leukocyte count (p = 0.897), or CD4+ count (p = 0.317) between the two groups. Conclusion: ARV therapy effectively improves hematological and immunological parameters in both HIV/AIDS patient groups. However, TB coinfection significantly hampers hemoglobin recovery after six months of therapy. These findings emphasize the need for enhanced monitoring and management strategies for HIV-TB coinfected patients.
