Abstract
Tuberculosis (TB) remains one of the leading causes of death globally, especially in developing countries like Indonesia. Diabetes mellitus (DM) has been identified as a comorbid condition that can exacerbate TB outcomes and affect renal function. Anti-tuberculosis drugs (OAT), particularly when administered in fixed-dose combination (FDC) or loose combination (Combipack), can have nephrotoxic effects due to prolonged use. This study aims to determine the differences in kidney function—measured by serum creatinine, ureum, creatinine clearance, and estimated glomerular filtration rate (eGFR)—between new pulmonary TB patients with DM treated with OAT FDC and Combipack at RSUP Persahabatan in 2024. A retrospective cross-sectional study design was used with purposive sampling from medical records of 65 eligible patients. Kidney function was analyzed before and after six months of treatment using appropriate statistical tests including independent t-test and Mann-Whitney based on data distribution. The majority of patients (81.54%) received FDC, and 18.46% received Combipack. Statistical analysis revealed no significant differences (p > 0.05) in changes of serum creatinine, ureum, creatinine clearance, and eGFR between the two groups. These findings indicate that both FDC and Combipack regimens do not significantly differ in their impact on renal function among new pulmonary TB patients with DM. Therefore, therapy selection between FDC and Combipack may be guided more by other clinical considerations than renal outcomes alone.
