Abstract

Chronic obstructive pulmonary disease (COPD) is a non-communicable disease that is a public health problem in Indonesia. The treatment recommended by the Global Initiative for Chronic Obstructive Lung Disease (GOLD) 2023 is long-acting beta-2 agonists (LABA) as maintenance therapy for stable COPD sufferers and inhaled corticosteroids (ICS) are also given to suppress inflammation. Maintenance therapy in COPD patients is carried out over a long period of time, so it is necessary to do a cost-effectiveness analysis (CEA) of several drug options to overcome treatment costs. This study aims to determine the CEA of combined COPD treatment, budesonide/formoterol and fluticasone/salmeterol, in clinical improvement using the COPD Assessment Test (CAT) score at Persahabatan Hospital, Jakarta. This study was conducted using observational analysis using a cross-sectional design on stable COPD patients in Persahabatan Hospital, Jakarta from January - December 2023. This study used 43 medical records from two groups of patients, budesonide/formoterol and fluticasone/salmeterol group therapy. The mean improvement in CAT scoring in budesonide/formoterol was 5.8 and in fluticasone/salmeterol was 4.6. The average direct medical costs for patients with budesonide/formoterol were IDR 748,854, while for patients with fluticasone/salmeterol were IDR 706,281. The ACER value based on clinical improvement between the budesonide/formoterol (Rp. 129.112/score) was smaller than the fluticasone/salmeterol (Rp. 153,539/score). Budesonide/formoterol is more cost-effective than fluticasone/salmeterol in improving CAT scores.
