Abstract
PT Alam Sinergi Gemilang is a company operating in the garment industry, which focuses on producing jersey products. Based on company data for September 2023 - February 2024, it was found that the total number of defects occurring in jersey products was 223 pcs with a total production quantity of 3138 pcs. Because of this, quality control is needed in the jersey production process at PT Alam Sinergi Gemilang using the FMEA and FTA methods. The results of FMEA data processing showed that there were 25 defect factors with 5 critical defect factors, namely the sewing operator lacked skill/experience, the sewing needle was broken/blunt, the heat press machine calibration settings were incorrect, the screen printing operator lacked skill/experience, the screen printing machine was not properly calibrated. This critical defect is obtained because the Risk Priority Number value for the defect is > the critical value which is 190 as the lower limit of critical defects. Next, the critical defects were processed using FTA and produced 65 intermediate events and 20 basic events (root causes). Improvement proposals are given using the 5W+1H method based on the FTA results of the critical causes of jersey product defects that occurred at PT Alam Sinergi Gemilang. The proposed improvements are expected to reduce the number of defects that occur by 7.1% and can be useful for the smooth production process at PT Alam Sinergi Gemilang.
