Abstract
PT Mahasuri Utama is a company engaged in clothing sewing services known as CMT (Cut, Make, and Trim) and apparel manufacturing. During the production process, it is inevitable that errors may occur, which in turn can cause defects in the product. The number of defects that occur, it is necessary to improve and control the production process. This research aims to design improvement proposals to overcome the defects that occur. The methods used in this research are the Failure Mode and Effects Analysis (FMEA), Fault Tree Analysis (FTA), and 5W+1H methods. The results of this study show that the most dominant product defects are perforated fabric 34%, untidy stitches 25.6% and uneven material color 24.3%. Therefore, one of the proposed improvements that can be made by the company is to monitor workers to ensure they work in a focused and thorough manner and apply each workstation to be a quality control at the previous workstation.
