Abstract
Background: Diabetes mellitus is a chronic disease that can last a lifetime. Type 2 DM results from ineffective insulin and is initially compensated by increased insulin production to maintain glucose homeostasis, but over time, insalin production decreases, causing insulin resistance. One of the complications of type 2 DM that is often encountered is Diabetic Foot Ulcers (UKD). Objective: To describe nursing care for diabetes mellitus clients with impaired skin integrity at RSU South Tangerang City starting from the assessment process, nursing diagnosis, nursing planning, implementation and nursing evaluation. Research Method: The research method used is a case study. Research Results: Nursing problems that arise in this case are impaired skin integrity related to peripheral neuropathy, instability of blood glucose levels related to insulin resistance, acute pain related to physical injurious agents. Conclusion: The results of the nursing assessment showed that the problem was partially resolved, namely the nursing diagnosis of impaired skin integrity related to peripheral neuropathy and the problem was resolved in Mr. S, namely instability of blood glucose levels related to insulin resistance, acute pain related to physical injurious agents.
