Abstract
The risk of falling is still an important aspect for health care service. Prevention of fall risk is not only done by nurses but also requires patient involvement. Patients and families need education to obtain information on how to prevent fall risk so that it can be implemented properly. The results of the study conducted in the Cemara 1 inpatient room showed that a number of patients and their families were less cooperative in preventing fall risk. Pre-test data using the Fall risk awareness questionnaire (FRAQ) showed that as many as 7 out of 15 patients (47%) had a poor perception of fall risk, so the authors provided an intervention in the form education and providing information about the risk factors of patient falls based on the concept of Patient Centered Care (PCC). This intervention was carried out during the patient's stay in the hospital. After 5 days of implementation, post test results from the same questionnaire showed that 6 out of 7 patients (86%) experienced an increased perception of the fall risk and also there was an increase in score by 91%. These results demonstrate the effectiveness of education and providing information in improving the patient's perception of the fall risk. In addition, patients and families seem willing to participate in preventing the risk of falling after this approach.
